MHY Family Services (MHY) is over 140 years old and a non-profit organization that provides support and services through educational and community-based programming all over Pennsylvania, primarily in Western PA. MHY strives to meet the changing needs of at-risk youth and their families through a practical and holistic approach to treatment. Youth and families have the opportunity to learn skills that help them manage and cope with trauma-based issues and to re-engage with home, school and community. SUMMARY OF INTENSIVE AFTERCARE SPECIALIST: Works with youth being discharged out of substitute care, or who are in risk of substitute care because of their threat to community safety. Coordinates with the youth, family, community supports, probation office, and school to ensure and maintain a successful reintegration into the community setting. Must be capable of identifying and recommending the supports necessary for a successful transition into the community. ESSENTIAL DUTIES AND RESPONSIBILITIES of the Intensive Aftercare Specialist include the following. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Other duties may be assigned. Initiates contact with parents, legal guardians, or other pertinent relatives following a youth's discharge from substitute care.
Where needed, prepares the family and make referrals for services such as mental health, substance abuse, social services, housing, and family treatment.
Participates in discharge planning to finalize the arrangements for a youth's release from a residential facility.
Participates in formal/informal court hearings and client reviews that require frequent testimony and accurate presentation of facts/supporting evidence.
Communicates progress of the youth to the probation office whenever appropriate.
Completes weekly progress sheets documenting the youth's compliance with services.
Completes all reports necessary to track youth's outcomes for the program.
Remains on call 24/7 for continued support of youth and family.
Maintains a flexible schedule to ensure that the youth and/or family as well as other stakeholders are served in accordance to MHYFS expectations/requirements.
Develops and maintains professional and collaborative relationships with youth, parents, community supports, staff, referring sources and other professionals.
Attends departmental and professional training, workshops, and conferences to ensure best practices. EDUCATION and/or EXPERIENCE Bachelor's Degree in Human Services field from an accredited college or university; minimum of two years of experience providing direct care to behaviorally disordered, socially maladjusted, or victimized children; experience working with a delinquent populations strongly preferred. Demonstrated ability to be a team player required, including a capacity to work well under pressure. Must be 21 years of age with current Act 33, 34 and FBI clearances. Current PA state driver's license with evidence of an insurable driving record and personal vehicle required. Must possess demonstrated verbal and written skills as well as strong PC skills, including common MS Office applications. Additional training or licensing as required by regulations, or as deemed necessary by the Executive Director or Director.
