Job DescriptionClient Care Specialist - Home Instead Senior Care
Overview Home Instead Senior Care is looking for an organized and
insightful individual to join our team in serving seniors, who wish
to age in place, and their families. This position requires someone
who is personable, able to identify and understand the needs of
others and take action, and quick to respond to the changing needs
of clients. The Client Care Specialist role is responsible for
managing multiple tasks at a time, must have excellent
communication skills, and work well in a team environment. Most
importantly, the Client Care Specialist is responsible for
overseeing the client/CAREGiver relationships and ensuring quality
of service. Objective The Client Care Specialist is responsible for
any activities relating to client care including care consultations
with potential clients and family members, matching clients with
their ideal CAREGiver based on their needs, overseeing
client/CAREGiver introductions, and following up on an ongoing
basis with clients and their families to ensure quality of care and
client/CAREGiver fit. This person will use a consultative sales
approach to determine each client's individual needs and create a
service plan tailored to these needs. The Client Care Specialist
will monitor this service plan and client needs through a series of
ongoing communications with both the client and their family to
ensure high-quality care, client satisfaction, and client
retention. It's pertinent that this person is also able to
recognize and evaluate opportunities to increase service hours and
further tailor the service plans as the client's needs change or
evolve. Responsibilities * Reflect the core values of Home Instead
Senior Care to ease burdens, keep promises, and preserve dignity
for our clients and their families. * Meet with potential clients
and family members to discuss their needs and provide tailored
solutions in the form of a service plan. * Conduct service
inquiries and care consultations as needed. * Maintain
communications and consistent follow-up schedule with potential
clients. * Work with other team members to coordinate aspects of a
client's care. * Conduct client/CAREGiver introductions with new
clients and CAREGivers. * Create and maintain an ongoing schedule
for communications and quality assurance meetings with all clients
and properly document all interactions. Minimum of one quality
assurance visit per quarter, per client. * Recognize and pursue
opportunities to modify service plans to best support the ongoing
needs of clients through a quality assurance program and
communications with CAREGivers. * Demonstrate open and effective
communication with clients, family members, CAREGivers, team
members and referral/care providers. * Resolve conflicts and
challenges as they arise for clients and family members by working
cooperatively with key team members, maintaining proper
documentation, and ensuring clients receive the services needed. *
Maintain regular attendance and adhere to all company and
state-mandated policies, procedures, and business ethic codes.
