Registered Nurse (RN) | Hospice Location: Meadville, PA Agency: Cynet Health Pay: $2,106 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/30/2026 About the Position Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM to 5:00 PM; call as needed 2-4 nights per month Experience: Minimum of one year as an RN in acute care, oncology, long-term care, hospice or home health License: Current license to practice nursing Certifications: BLS (American Heart Association) Must-Have: - Knowledge of terminally ill patients and their families, along with an understanding of hospice care - Ability to apply knowledge of special needs of hospice patients and families - Excellent patient assessment skills - Strong oral and written communication skills - Knowledge of community resources and general nursing practices Description: The Registered Nurse (RN) will provide direct patient care services to hospice patients. Supportive care will be provided to the patients' families. The RN will manage an interdisciplinary group consisting of support staff to enhance the quality of hospice care. They will case manage all assigned patient clinical records. The RN will ensure the highest quality of care is provided and documented according to the Plan of Care. Initial and ongoing assessments of the patient will be performed. The RN will help develop and implement the Hospice Plan of Care as prescribed by the physician. Close collaboration with other team members is essential to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services. This includes bereavement support for the patient and their family. Any changes in the patient's condition will need to be reported to the attending physician. A medical record will be maintained, which includes all interdisciplinary notes, Plans of Care, assessments, and other relevant information. Infection control measures and standard precautions will be practiced. The RN will monitor the scheduling of all staff and participate in quality assessment programs. Participation in the agency on-call schedule may be requested. The RN will promote core values consistently. This role is considered safety-sensitive. Required compliance training will be completed annually. Other assigned activities will be performed as needed. Attendance at staff meetings is expected. A high level of knowledge pertaining to pain management and symptoms in the dying process should be maintained. Routine supervisory visits of other team members will be conducted at least monthly. Ongoing reviews and follow-up on patient clinical records will ensure quality services are provided. Maintaining close contact with the patient’s family and/or significant others is crucial for continuity of care. The RN will have primary control and professional management of each patient. They will act as a liaison between the physician, patient/family, and the hospice team. Education for the patient, family, or caregivers will be provided concerning identified needs in the Plan of Care.
