everyone in our community. The Community Health Worker is
responsible for conducting enhanced outreach, HIV/STI/HCV testing,
and evidence-based intervention programming with gay, bisexual, and
other men who have sex with men (MSM) and linking them with HIV
preventative treatment and/or medical care. The Community Health
Worker will work toward developing the quality and effectiveness of
the program activities to ensure that annual program objectives are
met. As part of the essential functions of this role: The Community
Health Worker: Assists with development and implementation of the
annual outreach plan, which includes face-to-face and social media
strategies, goals and objectives for reaching the target population
Promotes and administers HIV/STI/HCV testing on site and at
community outreach events with a behavioral risk assessment
Conducts outreach according to strategies identified in the annual
plan, grant workplan and the enhanced outreach model. Refers
participants to to PEP, PrEP, HIV/STI/HCV testing and treatment,
care coordination and HIV medical care; assists participants with
appointment reminders if needed; accompanies participants to
appointments if needed Connects participants to SDOH efforts based
on identified barriers and available services to include supportive
counseling and MOCHA C.A.R.E.S. Provides 1:1 health education
around HIV/STI prevention and treatment for gay, bisexual,
transgender, and men who have sex with men who identify as persons
of color throughout priority populations. Qualified candidate: Will
have a high school diploma or equivalency preferred; personal or
professional experience with HIV/MSM communities and communities of

The actual rate of pay offered will be based on a variety of
individualized factors, including but not limited to a candidate’s
skills, relevant experience, educational background, internal
