Evaluates the client's level of function by applying diagnostic and prognostic muscle, nerve, joint and functional abilities testsEvaluates the home environment for rehabilitation and makes appropriate recommendations to increase the independence of the clientSupplies physical therapy services in the home as prescribed by a physicianAdministers manual therapeutic exercises; instructs, motivates and assists clients in non-manual exercises and functional activitiesAdministers traction, massage and treatments involving the application of physical agentsEvaluates, fits and adjusts prosthetic and orthotic devices and recommends any modifications to the physicianParticipates in the development, review, and updating of the care planObserves, records and reports any changes in the client's condition and response to therapy and makes suggestions for changes in the plan of care to the client's physician and Case ManagerPrepares clinical records (initial evaluations - including OASIS, care plans, progress notes, discharge summaries) and submits them for the client's clinical recordParticipates as a member of the interdisciplinary team in case conferences and in planning and evaluating the total treatment for the client and confers with interdisciplinary team members as neededAdvises and consults with the client/family and other agency personnelSubmits orders and documentation (clinical and payroll) on a timely basis and according to branch policyInstructs and supervises Home Health Aides or CNAs/STNAs, as applicable, in certain phases of physical therapy services they may to the clientInstructs the client/family in the use and care of physical therapy devices and on client's total physical therapy programInstructs and supervises Physical Therapy Assistants in certain phases of physical therapy services they may provide to the clientSupervises the Physical Therapy Assistant minimally as required by applicable state lawCompletes an on-site functional assessment, review and revision of the plan of care and assessment of utilization of community resources as required by clientAccessible to the Physical Therapy Assistant when a Physical Therapy Assistant is providing treatment to a clientCompletes other assignments and duties as requested and assigned to accomplish the goals and objectives of the CompanyWhat we'll love about you:
