Job Description:

Crossbridge Hospice seeks a passionate and dedicated Community Liaison to join our team, dedicated to providing outstanding end-of-life care. This role is vital in nurturing relationships with the community, patients, and their families, ensuring they receive compassionate support and comprehensive information about our services. Our ideal candidate will act as the bridge between our patients, the community, and the various healthcare professionals who contribute to their care. This role involves a combination of outreach, advocacy, and educational tasks aimed at fostering a supportive network for our clients at a critical time in their lives. Develop and maintain relationships with community organizations, healthcare providers, and local groups to advocate for patient needs and hospice services. Organize and participate in community events and presentations to raise awareness about hospice services. Support the coordination of patient care by facilitating communication between families and multidisciplinary teams. Provide timely and accurate information to patients and families regarding hospice services and end-of-life care options. Identify community resources and support systems for patients and families, assisting them in navigating healthcare services. Gather feedback from the community and patients to continuously improve services and outreach efforts. Strong interpersonal and communication skills Ability to work compassionately with diverse populations Excellent organizational and public speaking skills Proficiency in maintaining professional networks and partnerships Capability to handle sensitive information with confidentiality and care Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Certified in healthcare communication and/or patient advocacy is a plus
